How to submit a Healthy Hearts Referral
Send completed referral form via email to reception@hhearts.com.au, or fax 9387 5705
Alternatively, give the completed form to the patient and have them contact us directly on reception@hhearts.com.au or (08) 9387 2699
Once we receive your healthy hearts referral:
- We’ll contact the patient promptly to arrange their initial assessment
- If needed, we’ll work with your clinic to gather any additional medical information
- A comprehensive report and personalised care plan will be sent back to your clinic
